Requirements:

Each athlete must be signed up prior to arrival and all forms turned in to a specified administrator.
All players, regardless of age and ability will be required to wear a hockey helmet with a face mask in the shooting and stick handling areas.
Hockey gloves and sticks are required as well as workout attire with closed toe shoes.
Sharing water bottles will not be allowed, bring your own hydration fluid.

Expectation:

All athletes must understand and conduct themselves in a safe and controlled manner.

I have carefully read and fully understand all all provisions of this waiver of liability and release, understand that I have given up substantial rights by agreeing to it, and freely and knowingly assume the risk and waive my rights concerning liability as described in the COVID-19 Waiver.

Liability Waiver:

I, the undersigned, the parent of guardian of the athelete, a minor, does hereby approve my child's participation in Top Shelf Hockey Midwest and its sponsored activities. I hereby consent and acknowledge that my child will be subject to and shall assume the ordinary risks of such sponsored activities.

I further agree to hold harmless and release Top Shelf Hockey Midwest, its employees, staff members, agent, contractors and anyone associated with any activities sponsored by Top Shelf Hockey Midwest from and and all liability arising from such activities. I further agree to hold harmless and indemnify Top Shelf Hockey Midwest from and and all demands, claims and suits arising from such activities, including all judgments, costs and expenses, including attorney feed.
